This form is for parents/guardians to complete for their young people aged 7-18 years who wish to attend one or more Youth Network Clubs. You can now sign up to multiple clubs in one go. Those aged 16 and over can complete their own form below or this one.
You only need to complete ONE form per young person. Make sure you select all the clubs you wish them to attend
